We would like to know a little bit about the animals in your life. For each animal you have owned in the past five years, please tell us the type/size, how did you obtain this animal, and where are they now?
I understand and agree that submitting this application form does not automatically register me as a Humane Society of the North Bay volunteer, and that there may be certain qualifications I must meet, including the acceptance of established volunteer policies and procedures before I may begin volunteering.
I understand that the Humane Society of the North Bay may perform a basic background check on volunteers to determine that there are no cases of animal cruelty.
I certify that the statements made in this volunteer application are true and have been given voluntarily. I understand that this information may be disclosed to any party with legal and proper interest, and I release the Humane Society of the North Bay (HSNB) from any liability whatsoever for supplying such information. I also understand that I will not be paid for my services as a volunteer.
In consideration of the HSNB accepting my application or my child's for participation in their volunteer programs, I agree to release and hold harmless the HSNB from and against any and all loss, damage, claims, liability, costs, and expenses of any nature whatsoever, including without limitation, attorney's fees and disbursements, arising from or occasioned by my participation in the HSNB’s volunteer programs. I understand there are certain risks inherent in handling the animals, and I accept these risks.
Further, I agree to treat all animals, people, and property I come into contact with at HSNB with the utmost respect and professionalism. I will seek management help for any questions or situations beyond my knowledge or training. I understand that difficult decisions and situations may present themselves routinely at HSNB, and I will conduct myself with courtesy and respect toward staff, volunteers, and the public at all times.
I understand that it is a violation of privacy and breach of confidentiality to publicly discuss or disclose any information I may learn about an animal’s previous owner or situation. I agree to hold absolutely confidential all information that I may obtain concerning patrons or animals. I understand that an intentional or unintentional violation of confidentiality may result in disciplinary action, including termination of volunteer services and/or possible legal action by others (i.e. patrons).
I fully understand that the HSNB expects high standards of moral and ethical treatment of animals under its care. I agree to adhere strictly to these standards in my voluntary capacity and exercise compassion, care, and positive training methods with all animals.
I agree to advise my supervisor of relevant information regarding an animal or patron in a timely manner. I will communicate job-related problems, concerns, differences of opinion, conflicts, or suggestions to the Volunteer Coordinator.
I understand that my volunteer service is an integral part of the daily operations of the shelter and I agree to notify the Volunteer Coordinator immediately of any schedule changes or if I wish to discontinue my volunteer services.
I will adhere to HSNB’s COVID-19 Safety Guidelines:
• Perform a Health Self-Assessment prior to each volunteer shift, and before arriving on campus.
• Stay home if you are sick (fever, cough, sneeze, aches, etc.).
• Avoid close contact with people who are sick.
• Wash or sanitize your hands prior to starting your volunteer shift and throughout the day.
• Wear a mask / face covering during your entire shift. Due to the limited supply of PPE items, you must supply your own mask. If you do not bring one, you will be asked to leave the shelter.
• Stay 6 feet away from others as much as possible.
• Avoid touching your eyes, nose, and mouth – especially with unwashed hands.
I understand that anyone above the age of 65 and or at increased risk for more severe diseases if infected should consult with their physician before participating in any volunteer activity.
I understand that the HSNB reserves the right to terminate my volunteer status at any time, for any circumstances which, in the judgment of the Volunteer Coordinator, Shelter Manager or Executive Director, would make my continued service as a volunteer contrary to the best interest of the agency.
